Payments

Payment is due in full at the time of service. There is a $10 late payment fee for all payments not received at time of service.

Cancellation

We require a 48 hour notice of cancellation except in cases of extreme sudden emergency (as determined by IFD). If you cancel without a 48 hour notice, you will be charged a cancellation fee, which may be equal to your full fee of service. All fees must be paid in full prior to your next appointment.
